چکیده

This paper compares the effectiveness of normal saline and heparinised saline as a flushing solution in maintaining patency of arterial catheters. Use of heparin as a flushing solution in maintaining patency of intraarterial catheters has been a recognized practice. Due to its anticoagulation properties, heparin can cause side effects like heparin-induced thrombocytopenia in hypersensitive patients. The alternative, normal saline solution increases the accuracy of patients’ coagulation status and enhances safer clinical practice. A systematic search was conducted at Cochrane Library, Medline, CINAHL, OVID and Joanna Briggs Institute. Review period covered from 1990 to 2011. Methodological validity of included studies was evaluated using SIGN grading checklists from Scottish Intercollegiate Guidelines Network. There is inconsistent evidence and sample sizes were too small to allow statistical significance to be achieved. Meta-analysis was performed for 2 studies; results revealed that heparinised saline offers no added advantage over normal saline. There is insufficient evidence to support the use of heparinised saline for flushing arterial catheters. A rigorous research design is needed to determine variables known to influence patency of arterial catheters. Sound clinical judgement must be exercised in situations that require the need of heparinised saline.

Normal saline versus heparin for patency of central venous catheters in adult patients - a systematic review and meta-analysis

BACKGROUND Heparin saline (HS) is theoretically superior to normal saline (NS) for maintaining the patency of central venous catheters (CVCs), but the comparative efficacy of them remains controversial. The aim of this systematic review and meta-analysis was to assess the efficacy of NS versus HS in the maintenance of the patency of CVCs in adult patients. METHODS We searched PubMed, Embase a...

Finding a solution: Heparinised saline versus normal saline in the maintenance of invasive arterial lines in intensive care.

BACKGROUND We assessed the impact of heparinised saline versus 0.9% normal saline on arterial line patency. Maintaining the patency of arterial lines is essential for obtaining accurate physiological measurements, enabling blood sampling and minimising line replacement. Use of heparinised saline is associated with risks such as thrombocytopenia, haemorrhage and mis-selection. Historical studies...

Avoiding Heparinization of Arterial Line and Maintaining Acceptable Arterial Waveform After Cardiac Surgery: A Randomized Clinical Trial

BACKGROUND Invasive and continuous blood pressure (BP) monitoring is crucial after cardiac surgery. Accuracy of BP measurement mostly depends on patency of arterial catheter and acceptable waveform. Heparinized saline flush usually used for this purpose may be accompanied by potential heparin adverse effects. OBJECTIVES The aim of this study was to compare heparinized and non-heparinized sali...

Patency of arterial catheters with heparinized solutions versus non-heparinized solutions: a review of the literature.

For decades, the patency of arterial catheters has been maintained with heparin and recently health care providers have considered that the patency of arterial catheters may be maintained with a non-heparinized solution. This is an important consideration because patients could avoid exposure to heparin-associated risks. The purpose of this article is to determine evidence-based practice for th...

Switching from heparinized saline flush to normal saline flush for maintaining peripheral venous catheter patency.

BACKGROUND The practice of using heparinized saline as a flush fluid for maintaining peripheral venous catheter patency of hospitalized patients in Siriraj Hospital in Bangkok, Thailand is not evidence-based. OBJECTIVE To switch from heparinized saline flush to normal saline flush for maintaining peripheral venous catheter patency in the patients hospitalized to medical wards. METHODS Study...
